The ConcertoCare Population Health Program & Analytics Manager leads workflow and tool development, process improvement, implementation, and performance monitoring for key Population Health initiatives, including Case Management, Moments that Matter, Pharmacy, and Behavioral Health programs. Reporting to the Senior Manager of Population Health, this role serves as the organization-wide program manager for designated initiatives and partners closely with market-level population health teams and leaders to support achievement of nursing and case management outcomes.
In this capacity, the Manager develops and maintains enterprise-wide processes and programs in collaboration with Directors and Vice Presidents of Population Health to support consistent implementation across markets. The role also serves as a business owner in partnership with IT and Analytics teams to ensure tools and reporting solutions are designed to meet operational and clinical business needs. Key responsibilities include leading user acceptance testing and data validation activities, monitoring workflow functionality, and collaborating with cross-functional program leaders to strengthen implementation processes and reporting over time. This position also provides indirect leadership through the development and delivery of training and train-the-trainer programs to promote standardized execution across markets.

- Collaboration: Collaborates with Nursing & Population Health and Market Leadership to standardize and optimize Case Management and Market RN care delivery, ensuring seamless integration of roles within each market’s interdisciplinary team. Works closely with Clinical Informatics, IT and Analytic teams to define, design, test and implement solutions to support Pop Health and Nursing programs.
RESPONSIBILITIES
- Develop and implement processes, program measurement, performance improvement, and change management plans to support Population Health programs across markets
- Develop and implement policies, procedures and job aids to support consistent program rollout and operations across markets in collaboration with respective team leaders
- Lead designated programs at an organizational level. Design and implement programs and strategies to improve ConcertoCare performance in key Population Health and Nursing areas
- Program performance monitoring and improvement designed to address program performance concerns and drive continued improvement
- Ensure timely implementation of process and workflow changes in collaboration with respective team leaders
- IT business owner partnership to ensure program workflows are enabled by technology to support workflows, reporting, and ongoing KPI evaluation.
- Lead UAT and data validation for Population Health tech tools and reports.
- Ensure data integrity, meaningful metric utilization, and timely reporting to leadership and clinical teams
- Translate Population Health program requirements into scalable technology and reporting solution requirements, ensuring systems support data integrity, interoperability, and high-quality documentation
- Leads and participates in cross-functional project teams and serves as a subject matter expert to improve ConcertoCare’s processes and programs through UAT to ensure proper functioning of workflows
- Generates and analyzes reports and to monitor for proper data flow within clinical reporting platforms
- Designs and implements scalable onboarding and training programs for clarity and maintains all onboarding documents and training materials for Population Health teams
- Travels periodically for new market startup activities, or targeted other meetings

QUALIFICATIONS
- Current and active clinical licensure in good standing
- Bachelor’s degree required
- Master’s degree in technology or clinical informatics preferred
- Minimum of 5 years’ experience working in a clinical setting, with at least 3 years of population health related experience in a value-based care setting
- Minimum 2 years Clinical Informatics and/or Analytics experience required with preference for those with productivity and KPI tracking experience
- Demonstrated experience and success with clinical program design, implementation and leadership in a multi-location, preferably national geographic model
- Strong project management, analytical, and problem-solving skills, with a data-driven approach to optimization of workflows, processes, and implementation
- Demonstrated proficiency in data validation, reporting, and analysis, data driven decision making and issue identification and tracking
- Experience serving as business owner for tech solutions, including requirement development, solution testing, and data validation
- Detail oriented, with strong problem-solving and organizational skills
- Demonstrated highly effective written and verbal communication
- Highly professional, and able to respond effectively to rapidly changing circumstances both internal and external to the ConcertoCare organization
- Demonstrated ability to perform multiple concurrent tasks with minimal supervision and meet deadlines
- Ability to work in a fast-paced, dynamic environment and work well with others on a team
- Proficient in computer skills to include Microsoft Office Suite (Outlook, Excel, PowerPoint, Word)
- Expert-level proficiency in clinical documentation and reporting systems including EMRs such as eCW and reporting platforms such as Power BI
Base Salary/ Wage Range $95,000 to $110,000 with annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, experience, and geography, and may fall outside of the range shown.
