Practice Manager - Tennessee

As part of our market leadership, the ConcertoCare Practice Manager serves as the operations leader and is responsible for managing the performance of an interdisciplinary team within a value-based care framework. Key activities include oversight of team/pod performance to ensure high-quality care delivery, managing day-to-day market operations, and personnel management of medical assistants, community health workers, and care navigators. This position works closely with the Market Medical Director and other market leadership roles and reports to the Operations Lead.
This is a hybrid position based in Tennessee with occasional field visits. Must reside in Tennessee, preferably in or near Nashville, and be able to travel in state periodically to support or visit the field team.
WHAT OUR PRACTICE MANAGERS DO:
- Data-driven management of team/pod to achieve market goals and targets for key performance indicators, including access to care, patient engagement and retention, utilization and cost, quality/HEDIS, employee satisfaction and retention, financial, and operational metrics.
- Day-to-day operational market support, including management of team/pod schedules to meet patient and operational needs, oversight of clinical and operational task completion, oversight of team huddles, managing patient complaints and grievances, and overseeing vendors and clinical supplies.
- Day-to-day clinical market support, including monitoring urgent situations and being available to support the team in addressing urgent care needs, resolving barriers to care, preventing unnecessary ER/admissions, and successfully managing care transition episodes as needed.
- Personnel management of medical assistants, community health workers, and care navigators, including hiring, onboarding, training, management to role-based key performance indicators, ongoing performance management, and support for career development of new and current staff.
- Support the team/pod through hands-on shadowing, collaboration, guidance, and support.
- Monitor clinical operational workflows, ensuring adherence to standard procedures and identifying and acting on opportunities to improve workflow design and efficiency.
- Lead implementation of new processes, programs, and pilots, taking on the effective role of market super-user and trainer for systems, pilot programs, and market/organizational initiatives.
- Develop and lead meetings required for management, training, education, and team building.
- Participate in interviewing, hiring, and training of new market/pod team members.
- Ensure team adherence to compliance policies, procedures, and standards of conduct including all applicable laws and regulations.
- Coverage of team duties as required.
- Active participation in growth of the practice.
- Support vendor, and external facility and healthcare provider relationships.
- Create and submit Staff Assignments in EMR and have direct oversight of Clinical Competency for field nursing
- Control and manage the Medical Supply and Medication administration practices
- Review patient Eligibility Management
- Other duties as assigned.

QUALIFICATIONS:
- Associate degree required, Bachelor’s or Master’s degree preferred; healthcare-related specialization preferred (e.g. Healthcare Administration/Management, Public Health, Medical Informatics, etc)
- 3+ years of management/operations experience at care delivery organizations, health systems, or provider groups required (5 years preferred); experience in value-based care, home health, and multi-site operations a plus
- Experience working with interdisciplinary teams (i.e. clinicians and non-clinical teammates) with 3+ direct reports • Previous P&L ownership a plus
- Start-Up, market-entry, implementation, or turnaround experiences a plus
- Medical Assistant, Licensed Practical Nurse, or Registered Nurse experience preferred, but not required
- Proficiency in data analysis and knowledge of applicable regulations and state and federal laws.
- Ability and means to travel as needed promptly to locations that may have limited access to public transportation; proof of liability and property damage insurance on the vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions/reinstatements, DUI, reckless driving, or felony convictions within the last 7 years. Multiple violations and accidents within the last 5 years would be subject to review.
Base Salary/ Wage Range $90,000 to $105,000 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
