The Field RN delivers in-home primary care clinical services with the support of a multidisciplinary care team. Our nurses build meaningful relationships with patients and their caregivers, partnering with them to provide care aligned with what matters most to our patients. Key activities include in-home patient care, patient and caregiver education, care coordination, and panel support activities. This position reports to the Practice Manager, with clinical oversight from the Medical Director.
WHAT OUR FIELD REGISTERED NURSES DO:
- Provide direct nursing care to patients through routine and urgent in-home visits under the direction of a clinical practitioner.
- Perform health screenings to identify unmet needs and support care planning activities.
- Facilitate virtual visits in the patient's home to support care delivery under the direction of a remote clinical practitioner.
- Coordinate and support patient care by implementing care plans, ensuring timely completion of orders and referrals, and performing outbound calls and tasks at the direction of a clinical practitioner.
- Provide panel support, including chart preparation and after-visit care coordination.
- Engage patients in their healthcare journey, establish meaningful connections and trusting relationships with each patient, and provide care tailored to them.
- Deliver exceptional customer service to our patients, caregivers, and clinical partners, including proactive, timely, and comprehensive communication from a holistic patient-centered perspective.
- Clinical skills and procedures: Blood draws, laboratory specimen collection, medication and vaccine administration, foley catheters, wound care, removal of sutures/staples, basic hearing and vision tests, home safety evaluations, and Quantaflo.
- Accurate and timely documentation in the medical record of all patient care support activities.
- Clinical supervision for licensed practical nurse (LPN) and/or medical assistant (MA) activities as requested.
- Integration of clinical practice, data dashboards, and operational processes that support achievement of company performance goals into daily clinical approach and workflows.
- Promotion of and participation in patient engagement and experience initiatives and efforts.
- Active participation in team huddles and organizational meetings.
- Adherence to compliance policies, procedures, and standards of conduct, including all applicable laws and regulations.
- Other market-specific duties as assigned.

QUALIFICATIONS:
- Bachelor’s degree in Nursing and/or commensurate clinical experience.
- Hold an active, unrestricted RN license in the state where you will practice.
- Current BLS certification required.
- IV or blood draw certification or relevant work experience.
- Clinical triage, urgent care, or emergency medicine experience preferred.
- Clinical experience caring for aging adults and adults with complex care needs is required.
- Ability to conduct independent patient home visits.
- Ability to travel 75%+ of the time within an assigned area.
- Previous experience with managed care and/or value-based healthcare delivery preferred.
- Average to advanced computer and software skills required.
- Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions/reinstatements, DUI, reckless driving, or felony convictions within the last 7 years. Multiple violations and accidents within the last 5 years would be subject to review.
Base Salary/ Wage Range $36.00 to $41.00 plus a performance-based annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
